We now have Vision Benefits!
Vision benefits covered included professional fees, materials,
lenses and frames. The maximum allowance for all benefits is
$200.00 per employee and covered dependent per calendar year.
Frames and contact lenses are available every other calendar
year.
You must pay for the services and send your receipt with a
completed vision form to the National Asbestos Workers Medical
Fund for reimbursement.
